Time for your family

The Federal Family and Medical Leave Act (FMLA) provides eligible employees up to 12 weeks of unpaid, job-protected time off (in a rolling 12-month period).

You can take Family and Medical Leave, with a medical professional's certification, for the following reasons:

  • A personal serious health condition that makes you unable to work or perform one or more of the essential functions of your position
  • Bonding time with your new child (including an adopted child or foster child)
  • Caretaking responsibilities for your spouse, child, domestic partner, or parent with a serious health condition
  • Caretaking responsibilities for your spouse, child, parent or next of kin who is in the Armed Forces and requires medical treatment from injuries that happened while in the line of duty
  • Certain qualifying reasons resulting from your spouse, son, daughter, or parent being on active duty in the Armed Forces

Eligibility

You may be eligible for Family and Medical Leave, if you have at least 12 months of employment with Gulfstream, and have worked at least 1,250 hours in the last 12 months.

FML Schedules

FMLA may be available on a continuous, or intermittent basis.

Initiating and reporting your FML Claim

Contact Sedgwick, the FML administrator, at 800-416-1808 or through their secure online portal mysedgwick.com at least 30 days before your leave begins or within seven days of your first absence. Intermittent time-off must be reported within 2 days of the absence.

Plan Administration

Sedgwick administers both family medical leave and short-term disability for Gulfstream. To report your disability claim or leave of absence, or for information about your claim, call Sedgwick at 800-416-1808.
