Your medical plan coverage at a glance

Anthem BCBS is the administrator of our four medical plans and the provider network. All medical plan options have in-network and out-of-network coverage. You pay less when you go to an in-network provider due to their contracted, lower rates with Anthem BCBS. Anthem also provides virtual care options, care management and utilization management services – which includes precertification of surgery, hospitalization, and certain medical procedures.

Prescription drug coverage is included in the medical plan. Visit the prescription drug benefits page for details.

As an Anthem BCBS member, employees qualify for discounts on products and services that help promote better health and well-being. To find the discounts available to you, log in to your Anthem account, choose 'Care' and select 'Discounts'.

This plan comes with a health reimbursement account (HRA). Here are a few of the key features:

  • Gulfstream will contribute $400* to your HRA. Only Gulfstream contributes to this account.
  • You can use this money to pay your covered medical and prescription expenses, but it cannot be used for dental or vision expenses. Use the funds in the your HRA to pay for covered expenses throughout the year, such as doctors’ visits or eligible prescription drug expenses. The account comes with a debit card or you can submit receipts for reimbursement.
  • Prescription drugs are not subject to the calendar year deductible. The deductible does apply to all other non-preventive services.
  • This plan has an aggregate deductible, which means if you cover dependents, the family deductible must be met by one or any combination of family members before the plan will pay for services subject to the deductible.
  • Your HRA balance does not roll over. You have 90 days after the end of the plan year to submit any outstanding claims that were incurred during the plan year.
  • Employees who are enrolled in TRICARE or Medicare, which makes them ineligible for an HSA, are eligible for an HRA and may enroll in this plan.

This plan comes with a health savings account (HSA). Here are a few of the key features:

  • Gulfstream will contribute seed money to your HSA; $600* for single coverage and $1,200* for family coverage.
  • You can use your HSA to pay for medical, dental or vision expenses.
  • This plan has an aggregate deductible, which means if you cover dependents, the family deductible must be met by one or any combination of family members before the plan will pay for services subject to the deductible.

Employees who are enrolled in Medicare part A or B, Tricare, or a spouse’s non-high deductible medical plan are not eligible to make or receive HSA contributions per IRS regulations and should not enroll in this plan.
